Wound Debridement in Singapore: What Patients Should Know

Many of us end up accidentally sustaining various wounds in our everyday lives. Wound debridement is a core part of wound management, especially for chronic wounds that may be at risk of infection or other complications. Some wounds are superficial and heal on their own, while others require medical attention to ensure proper healing.

What is Wound Debridement?

Wound debridement is a wound treatment process that involves cleaning a wound and removing any dead or unhealthy (nonviable) skin, tissue, or foreign material in the wound. It can be performed through various conservative and surgical means to promote new tissue growth, reduce inflammation, and enable treatments to further mitigate the risk of infection and support wound healing.

Dead and unhealthy tissue can easily “trap” a wound, preventing it from healing properly on its own. This tissue may also harbour harmful bacteria that utilise available nutrients and oxygen to proliferate, potentially exacerbating the wound’s condition. As such, thorough and consistent wound debridement is necessary to ensure your wound recovery can proceed with little to no complications.

Types of Wound Debridement

The type of debridement used to treat your wound will depend on its condition and type, your age, your general health, and other factors. A wound care specialist will assess your wound and determine the most suitable debridement method

Biological Debridement

Also known as larval therapy or maggot debridement therapy, this type of wound debridement uses sterile maggots placed in a mesh bag that remove dead tissue and certain bacteria. They also reduce the risk of infection by secreting antibacterial substances. This therapy is primarily recommended for wound debridement of large wounds or those infected with antibiotic-resistant bacteria, though it may also be indicated as an alternative to surgery in some cases. The larvae will be left there to treat the wound for several hours to a few days and will be replaced twice a week, depending on the wound care specialist’s assessment.

Enzymatic Debridement

Also known as chemical debridement, this process relies on chemical enzymes derived from microorganisms to dissolve nonviable tissue and promote wound healing. It is often applied topically to the wound once or twice daily, working from the bottom up to loosen collagen fibres that hold onto the necrotic tissue. Care must be taken when using this method, as it may also cause irritation to surrounding tissue.

Autolytic Debridement

This type uses your body’s enzymes and natural fluids to soften, break down, and dissolve nonviable tissue, promoting wound healing while leaving healthy tissue unaffected. It is commonly prescribed for smaller, uninfected wounds, as the process is slow and may increase the potential risk of infection in larger wounds. It may also be used as a preparatory treatment for other types of debridement.

Mechanical Debridement

The most common type of wound debridement, mechanical debridement, involves removing unhealthy tissue from the wound using external force. It may include hydrotherapy, in which water is used to wash away old tissue, and wet-to-dry dressings, which place a moist, sterile bandage on the wound and are then removed when dry to remove dead tissue adhered to its surface. This type may cause some pain or discomfort and may injure healthy tissue if not prepared carefully.

Surgical/Sharp Debridement

This debridement involves removing nonviable tissue, either using scalpels, curettes, or scissors (conservative sharp debridement) or with surgical instruments (surgical sharp debridement). While generally considered the gold standard for wound debridement, sharp debridement is not always considered due to its potential risks, such as bleeding or injuries to other structures. Moreover, it may not be suitable for treating all wound types, such as Active bleeding disorders. As such, only qualified wound specialists familiar with sharp debridement techniques should perform this procedure, if necessary.

How is Wound Debridement Performed?

Initial Assessment

A wound care specialist will first assess your wound and perform a comprehensive physical exam to determine a suitable wound debridement plan. It may also include a review of your medical and medication history to rule out other conditions or risk factors. They will then discuss your debridement and treatment options to help you understand the procedure and manage expectations.

Wound Cleaning

The wound is carefully cleaned with antiseptics or saline to reduce the risk of complications. Local anaesthesia may be administered if necessary to ensure your comfort during this process.

Wound Debridement

Depending on the type of wound debridement, the procedure may take several minutes or hours. You may be given a local or general anaesthetic to ease any pain or discomfort during the procedure. After the procedure, you may be monitored for a short time to ensure the debridement was performed correctly and that you are feeling well post-debridement.

What to Expect After a Wound Debridement is Performed

After debridement, the wound is carefully cleaned and then dressed with a medical dressing to reduce the risk of infection. The type of dressing used may also depend on the debridement you underwent (e.g., hydrogels for autolytic debridement). You will also be given aftercare instructions to help you manage and care for your wound at home. Follow-up appointments may also be scheduled to keep up with your wound’s recovery and provide advice or small changes to your wound management. You may also need to keep track of your wound’s condition while at home.

  • Change your dressing often to prevent the risk of infection.
  • Keep the wound and dressing dry and clean at all times.
  • Always wash your hands before and after touching the wound.
  • Minimise pressure on the wound.
  • Enjoy a healthy, well-balanced diet to support your nutrition and recovery.
  • Cut down on alcohol and quit smoking to support your wound healing.
  • Take note of changes in pain levels, redness, swelling, and other potential warning signs.
